ARTICLE I: NATURE OF THIS AGREEMENT
This Membership Agreement (the “Agreement”) is entered into between Anglican Care (the “Ministry”), a faith-based Health Care Sharing Ministry, and the individual whose name appears above (the “Member”). This Agreement sets forth the terms, conditions, rights, and obligations of membership in Anglican Care.
Anglican Care is a 501(c)(3) nonprofit religious organization that facilitates the voluntary sharing of medical needs among Members who share common ethical and religious beliefs. Anglican Care is not an insurance company. This Agreement is not a policy of insurance. Anglican Care does not guarantee that any medical need will be shared, and Members do not have a contractual right to receive payment for any medical expense. Participation in Anglican Care is entirely voluntary.
ARTICLE II: DEFINITIONS
"Ministry": refers to Anglican Care, a Health Care Sharing Ministry organized as a nonprofit religious organization.
"Member": refers to an individual who has been accepted into membership, has signed this Agreement, and remains in good standing.
"Monthly Contribution": refers to the voluntary amount a Member contributes each month to participate in the sharing of medical needs.
"Medical Need": refers to a documented medical expense submitted by a Member that meets the eligibility criteria set forth in the Sharing Guidelines.
"Sharing Guidelines": refers to the published rules and criteria that determine which medical needs are eligible for sharing among Members.
"Sharing": refers to the voluntary process by which Members' Monthly Contributions are used to assist other Members with eligible Medical Needs.
"Good Standing": refers to a Member who is current on all Monthly Contributions and is in compliance with this Agreement.
ARTICLE III: MEMBERSHIP ELIGIBILITY & QUALIFICATIONS
To be eligible for membership in Anglican Care, an applicant must meet all of the following criteria:
(a) Profess faith in Jesus Christ as Lord and Savior and agree with the Statement of Faith as published by the Ministry.
(b) Regularly attend a Christian church or fellowship community.
(c) Agree to live a lifestyle consistent with biblical teachings, including abstaining from the use of tobacco, illicit drugs, and the abuse of alcohol.
(d) Complete and submit a Membership Enrollment Form with all required information.
(e) Sign this Membership Agreement and agree to all terms set forth herein.
(f) Maintain current Monthly Contributions as specified in the Member's selected program.
3.1 Statement of Faith
Members affirm the following beliefs: (1) There is one God, eternally existing in three persons: Father, Son, and Holy Spirit. (2) Jesus Christ is the Son of God, born of a virgin, who lived a sinless life, died for our sins, and was raised from the dead. (3) The Bible is the inspired and authoritative Word of God. (4) Salvation is by grace through faith in Jesus Christ. (5) Members are called to bear one another's burdens in accordance with Galatians 6:2.
3.2 Membership Approval
The Ministry reserves the right to accept or decline any application for membership at its sole discretion. Approval of membership is not guaranteed. The Ministry may request additional information or documentation before making a determination.
ARTICLE IV: MEMBER RESPONSIBILITIES & CODE OF CONDUCT
Members agree to the following responsibilities:
4.1 Provide accurate and truthful information on all enrollment forms, medical need submissions, and correspondence with the Ministry.
4.2 Notify the Ministry within thirty (30) days of any change in address, phone number, email, marital status, household composition, or program selection.
4.3 Submit Monthly Contributions on or before the due date each month.
4.4 Use medical providers and services wisely and cost-effectively, as all Members' contributions are used to share one another's needs.
4.5 Submit medical needs for sharing consideration promptly and with all required documentation.
4.6 Treat Ministry staff, volunteers, providers, and fellow Members with respect and Christian courtesy at all times.
4.7 Maintain a lifestyle consistent with the Statement of Faith and the values of the Anglican Care community.
4.8 Refrain from misrepresenting Anglican Care as an insurance product or implying guaranteed coverage for any medical expense.
ARTICLE V: HOW SHARING WORKS
5.1 Anglican Care facilitates the voluntary sharing of medical needs among Members. Monthly Contributions are collected and used to share eligible medical needs of other Members.
5.2 Sharing is not guaranteed. The Ministry does not guarantee that any specific medical need will be shared, nor does it guarantee the amount or timing of any sharing. Sharing is contingent upon the availability of contributions and the eligibility of the need under the Sharing Guidelines.
5.3 Eligible medical needs are determined by the Sharing Guidelines, which are published separately and may be updated from time to time. The Sharing Guidelines control which medical services, treatments, and conditions are eligible for sharing.
5.4 Pre-existing conditions: Medical needs related to conditions that existed prior to the Member's effective date of membership are eligible for sharing after a waiting period of twelve (12) consecutive months of continuous membership, subject to the Sharing Guidelines.
5.5 Per-need sharing limits and annual sharing limits may apply, as specified in the Sharing Guidelines for each program level.
5.6 What Is NOT Eligible for Sharing
The following are generally not eligible for sharing, subject to the Sharing Guidelines:
- • Elective cosmetic procedures or surgeries
- • Abortions, except when necessary to save the life of the mother
- • Fertility treatments, in vitro fertilization, or artificial insemination
- • Treatment for substance abuse or addiction beyond a defined limit
- • Experimental or investigational treatments not approved by the FDA
- • Self-inflicted injuries or injuries resulting from the commission of a crime
- • Expenses covered by or eligible for reimbursement from another source
- • Routine dental, vision, or hearing care (unless covered by an add-on program)

ARTICLE VI: CONTRIBUTION OBLIGATIONS
6.1 Each Member shall pay a Monthly Contribution in the amount corresponding to the Member's selected program, age category, and household size, as set forth in the published contribution schedule.
6.2 Monthly Contributions are due on the first (1st) day of each month. A grace period of fifteen (15) days shall apply. Contributions not received by the sixteenth (16th) of the month shall be considered late.
6.3 Late contributions: A Member whose contribution is more than thirty (30) days overdue shall have their membership suspended. Suspended Members are not eligible to submit medical needs for sharing during the period of suspension.
6.4 Payment methods: Monthly Contributions may be made by electronic funds transfer (ACH), credit or debit card, or check, as made available by the Ministry.
6.5 Contribution amounts are subject to change. The Ministry shall provide Members with no less than sixty (60) days written notice of any change to the contribution schedule.
ARTICLE VII: MEDICAL NEED SUBMISSION & SHARING PROCESS
7.1 Members must submit medical needs for sharing consideration within ninety (90) days of the date of service. Needs submitted after this period may be ineligible for sharing at the Ministry's discretion.
7.2 Each medical need submission must include: (a) the Member's name and Membership ID; (b) the provider's itemized bill or explanation of benefits; (c) diagnosis and treatment information; and (d) any other documentation reasonably requested by the Ministry.
7.3 The Ministry shall review each submitted need for eligibility under the Sharing Guidelines. The Ministry may request additional information or clarification from the Member or the medical provider.
7.4 Upon verification of an eligible need, the Ministry shall process the sharing of that need in accordance with the Sharing Guidelines and the Member's program level. The Ministry shall make reasonable efforts to process needs within sixty (60) days of receiving all required documentation.
7.5 Members are responsible for negotiating with medical providers and arranging payment arrangements where sharing is delayed, incomplete, or denied. The Ministry is not a party to the Member's financial obligations to providers.
7.6 If a need is denied, the Member may appeal the decision in accordance with the dispute resolution process set forth in Article IX.
ARTICLE VIII: CANCELLATION & TERMINATION
8.1 Voluntary Cancellation by the Member
A Member may cancel their membership at any time by providing written notice to the Ministry. Cancellation shall be effective on the last day of the month in which notice is received. Monthly Contributions paid for the current month are non-refundable. The Member shall remain responsible for any contributions due through the effective date of cancellation.
8.2 Cancellation for Non-Payment
If a Member's Monthly Contribution is more than sixty (60) days overdue, the Ministry may terminate the membership. The Member shall receive written notice of the intent to terminate at least fifteen (15) days before the effective date of termination. The Member may reinstate their membership by paying all past-due contributions before the effective date of termination.
8.3 Termination by the Ministry
The Ministry may terminate a membership for cause, including but not limited to: (a) material breach of this Agreement; (b) submission of fraudulent or intentionally misleading information; (c) failure to maintain the qualifications for membership; (d) conduct inconsistent with the Statement of Faith or the values of the Ministry. The Ministry shall provide written notice of termination and the reasons thereof. Termination is effective immediately upon notice in cases of fraud or material misrepresentation.
8.4 Effect of Cancellation or Termination
Upon cancellation or termination: (a) the Member's eligibility to submit medical needs for sharing shall cease; (b) Monthly Contributions already paid are non-refundable and non-transferable; (c) medical needs incurred prior to the effective date but not yet submitted shall be processed if submitted within thirty (30) days of the effective date and otherwise meet eligibility requirements; (d) the Member may reapply for membership after six (6) months, subject to all eligibility requirements and a new waiting period for pre-existing conditions.
ARTICLE IX: DISPUTE RESOLUTION
9.1 Members and the Ministry agree to resolve all disputes in a manner consistent with biblical principles, as set forth in Matthew 18:15-17 and 1 Corinthians 6:1-8.
9.2 Informal resolution: The Member shall first seek to resolve any dispute by contacting the Ministry in writing. The Ministry shall respond within thirty (30) days and shall make a good-faith effort to resolve the matter.
9.3 Mediation: If the dispute is not resolved informally, the parties shall submit the matter to Christian mediation through a mutually agreed-upon mediator or an organization such as Peacemaker Ministries or the Institute for Christian Conciliation. The cost of mediation shall be shared equally by the parties.
9.4 Arbitration: If mediation does not resolve the dispute, the parties shall submit to binding Christian arbitration in accordance with the Rules of Procedure for Christian Conciliation. The arbitrator's decision shall be final and binding on both parties. Judgment upon the award may be entered in any court of competent jurisdiction.
9.5 Class action waiver: The Member agrees that any dispute shall be resolved on an individual basis and not as part of a class or representative action. The Member waives the right to participate in any class action against the Ministry.
9.6 Governing law: This Agreement shall be governed by the laws of the State of Florida, without regard to its conflict of laws provisions. Any arbitration or legal proceeding shall be conducted in the State of Florida.
ARTICLE X: GENERAL PROVISIONS
10.1 Amendments: The Ministry reserves the right to amend this Agreement and the Sharing Guidelines at any time. Amendments shall become effective thirty (30) days after publication to Members. Continued participation after the effective date constitutes acceptance of the amended terms.
10.2 Severability: If any provision of this Agreement is found to be unenforceable or invalid, that provision shall be severed and the remaining provisions shall remain in full force and effect.
10.3 Entire Agreement: This Agreement, together with the Membership Enrollment Form, the Sharing Guidelines, and the Statement of Faith, constitutes the entire agreement between the Member and the Ministry.
10.4 No Assignment: A Member may not assign or transfer their membership or any rights under this Agreement to any other person or entity.
10.5 Notices: All notices to the Ministry shall be sent to Anglican Care, at the address published on anglicancare.org. Notices to the Member shall be sent to the address or email on file.
10.6 No Waiver: The failure of the Ministry to enforce any provision of this Agreement at any time shall not constitute a waiver of that provision or the right to enforce it at a later time.
10.7 Independent Contractors: Brokers, agents, and representatives who assist Members with enrollment are independent contractors and not employees or agents of the Ministry. The Ministry is not liable for their acts or omissions.
10.8 Compliance with Laws: The Ministry operates as a Health Care Sharing Ministry in accordance with applicable state and federal laws. The Ministry does not operate as an insurance company and is not licensed as such. This Agreement is not a contract of insurance.
ARTICLE XI: ACKNOWLEDGMENT & AGREEMENT
By signing below, the Member acknowledges and affirms that:
(1) The Member has read this Membership Agreement in its entirety and understands all of its terms and conditions.
(2) The Member understands that Anglican Care is a Health Care Sharing Ministry and does not operate like traditional health insurance.
(3) The Member understands that participation is voluntary and that there is no guarantee that any medical need will be shared.
(4) The Member understands that Monthly Contributions are voluntary gifts that are used to share the medical needs of other Members.
(5) The Member agrees to abide by the Sharing Guidelines, the Statement of Faith, and all terms of this Agreement.
(6) The Member understands that this Agreement may be amended by the Ministry with thirty (30) days notice.
(7) The Member is signing this Agreement freely and voluntarily, without duress or coercion.
