Complete the enrollment process yourself through our secure portal. It’s quick, easy, and guided every step of the way.
What You’ll Need to Enroll
- Personal information: Full legal name, date of birth, gender, marital status, SSN (last 4), contact details for you and any dependents
- Address & household info: Street address, city, state, ZIP, occupation, employer
- Program selection: Choose your tier (Foundational, Essential, or Complete) and your MRA
- Spouse & dependents: Names, dates of birth, and relationships for household members
- Health information: Pre-existing condition disclosure (36-month lookback), tobacco use, height/weight
- Statement of Faith: Agreement with our faith-based community principles
- Payment method: ACH, credit/debit card, or check for your monthly contribution
The entire enrollment process takes about 15–20 minutes. There are no medical questionnaires beyond a standard health disclosure, and no health underwriting.

Anglican Care is currently available in 24 states with safe harbor provisions for Health Care Sharing Ministries. If your state isn’t listed, we may still be able to help — contact us for more information.
